Die Moglichkeit, die Gehirndurchblutung beim Menschen am intakten Schadel regional und quantitativ messen zu konnen, ist als ein wichtiger Fortschritt fUr die Diagnostik und Therapie der cere bra len Durchblutungsstorungen zu werten. FUr die Anwendung dieses Untersuchungsverfahrens in der Neurologie ist aus lokalisations diagnostischen GrUnden eine moglichst weitgehende meBtechnische Ausrichtung an den anatomischen Gegebenheiten des Gehirns zu fordern. Diese Anforderung konnte mit einem MeBplatz verwirklicht werden, der innerhalb einer GroBhirnhemisphare nach Lage, GroBe und Form in engen Grenzen definierte MeBraume erfaBt, eine Zu ordung anatomisch bestimmter Hirnregionen mit bekannter GefaB versorgung zu diesen MeBraumen ermoglicht und durch Kombination mit der cerebralen Angiographie eine Korrelation von ortlicher cerebraler Durchblutung mit dem morphologischen Bild der Gehirn gefaBe erlaubt. Das Untersuchungsverfahren hat in den letzten zehn Jahren zuneh mend Eingang in die klinische Neurologie gefunden. Es sind bis heute im wesentlichen vier Anwendungsgebiete hervorzuheben. Das erste Indikationsgebiet stellen die intracraniellen, vaskularen Erkranungen mit angiographisch gesicherten Total-, Teil- oder AstverschlUssen der A. cerebri media, anterior oder posterior dar sowie die intracerebralen Angiome, die Aneurysmen mit und ohne Subarachnoidalblutung und die Thrombosen der intracraniel len Venen und Sinus. Bei diesen Erkrankungen vermag die regionale Gehirndurchblutungsmessung Uber den angiographischen Befund hin aus AufschluB Uber Starke und AusmaB der ischamischen Gewebs schadigung und die Durchblutungsanderung wahrend der Heilungs phase zu geben, die fUr die Prognose dieser Erkrankungen von Be deutung sind.
